Intradistrict Transfer Application Request (within HLPUSD)
The Intradistrict Transfer Application is for residents of the Hacienda La Puente Unified School District wanting to request to attend a different school within Hacienda La Puente Unified School District. Intradistrict Transfer Applications are for Special Circumstances. All Intradistrict applications will be completed and submitted electronically.

Interdistrict Transfer Application (outside HLPUSD)
The Interrdistrict Transfer Application is for residents of the Hacienda La Puente Unified School District wanting to request to attend a public school district other than their home district. All Interdistrict applications will be completed and submitted electronically.

INTERDISTRICT TRANSFER APPLICATION
The online form will require you to provide a letter of reason for leaving and any documents to support your request. Once the form is complete, parents will receive an email confirmation of submission. This informational email does NOT constitute an acceptance of release from HLPUSD, but will provide you with verification that it has been submitted along with your answers to questions on the form.
Processed applications will be returned to parents via electronic mail to the email address provided in the application.
All transfer requests will need to be approved by the Director at Student and Family Services, phone (626) 933-4335.
Open Enrollment (Intradistrict - Open Enrollment application)
Open Enrollment will open on January 1, 2027 and close on February 1, 2027.
All HLPUSD students generally attend the school where their residency has been established. HLPUSD residents also have the opportunity to submit a School of Choice application during Open Enrollment. HLPUSD Families that apply for Open Enrollment this year, will be placed in the applicant pool for their school of choice for the 2026-2027 school year.
All Open Enrollment applications will be done electronically.

This will only be active during the Open Enrollment period. Must complete the application by February 1, 2027.
*Please note this is not for new enrollments or students on Inter-district Permit from outside the district. New student enrollment (TK-12) for 2026-27 school year begins on January 20, 2026.

Shared Residency Affidavit
This form is to be completed if proof of residency verificiation cannot be provided due to the fact that the parent and child(ren) are sharing a home with another person full time. This affidavit must be re-certified annually.
Parent/Guardian Acknowledgment Form
This annual parent/guardian acknowledgment form provides important information about student and parent rights, district policies, and applicable California Education Code requirements.
